In certain circumstances, INTEGRA acts as a third-party administrator, business associate, or service provider for employer-sponsored benefit plans and may receive, use, maintain, or disclose Protected Health Information ("PHI") as permitted or required by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), related regulations, applicable agreements, and other governing laws.

When acting in these capacities, INTEGRA handles PHI in accordance with applicable Business Associate Agreements, plan documents, organizational policies, and legal requirements governing the privacy and security of health information.

INTEGRA may share information with carefully selected third-party service providers that support benefit administration and business operations. These providers may include claims processing partners, pharmacy benefit management partners, benefit administration vendors, healthcare service partners, cloud hosting providers, technology service providers, customer support providers, payment processing providers, and other contractors performing authorized services on behalf of INTEGRA.

INTEGRA administers various health plan benefits, including but not limited to medical, pharmacy, flexible spending accounts (FSA), and health reimbursement arrangements (HRA). Retention periods vary based on the type of information, plan requirements, and applicable laws, including but not limited to HIPAA, ERISA, and IRS regulations.

In general, information is retained for the duration of plan participation and for any additional period required to fulfill audit, reporting, and recordkeeping obligations.

Upon receipt of a verified request, INTEGRA will take reasonable steps to delete or de-identify personal information, unless retention is required or permitted for legitimate business or legal purposes, including plan administration, claims processing, and compliance with applicable laws and regulations.
